Job Requirements:
Symphony Wellness Coordinator reports to the Executive Director. Must possess the skills (assessment, interviewing, counseling and writing) to meet the health needs of our residents; collaborate and plans with other team members to address health outcomes for residents. Follows up on all resident and family enquiries, takes accountability to ensure resident health care needs are met. Understands and embraces the philosophy of validation and reminiscing for our residents with Alzheimer’s or related dementia. Has a clear understanding and demonstrated ability to provide resident centered care vs. task centered care.
Requirements:
- 2-3 years experience providing direct care
- Prior Supervisory experience
- Basic knowledge and understanding of Alzheimer's Disease and related dementias and a desire to serve residents and their families.
- Proactively schedule and maintain staffing based upon the occupancy and specific are needs of the residents to ensure appropriate levels of staffing.
- A Registered Practical Nurse with a current certificate of competence with the College of Nurses or governing body for the region in which they practice.
- Proof of medication administration education/ competency
- Excellent interpersonal and communication skills
- Possess complete understanding and application of the Resident’s Bill of Rights
- Clear understanding of resident centered care
- Leadership ability
- Excellent organizational skills
- Strong team orientation
- Trained in C.P.R. and First Aid
- Must be able to communicate in English (written, verbal, reading)
- Computer literate with Outlook, Excel, Word as well as an aptitude and willingness to learn other job specific software
Job Qualifications:

- Must have a valid Ontario drivers license
- Must be licensed and in good standing with the Ontario College of Nurses
- Must be available to work weekends

- Due to our high level of residents with dementia; your tattoos and piercings must either remained covered by clothing or are discrete. Any other external body decoration, including hair, must not be frightening to a frail senior with dementia.

- You must be in good physical health as this job requires lifting, sitting, standing and walking.
