Program Background:

Located in Ottawa, The John Howard Society of Ottawas (JHSO) Justice Transition Home (JTH) is a hybrid group-home-apartment program with the capacity to support 5 clients exiting the provincial correctional system in Ontario who have a developmental disability & mental illness (dual diagnosis) and/or challenging behaviour that requires specialized supports to meet their prescribed requirements. Individuals will have restrictions associated with their release that pose barriers to access other MCCSS-funded Developmental Services and Residential options. In order to be considered for the JTH, all other potential housing options must have been exhausted and no existing or new appropriate local resource solution is available.

In partnership with Ottawa Probation and Parole, The Royal Ottawa Mental Health Centres Flexible Assertive Community Treatment Team for Persons Dually Diagnosed (FACT-DD), the Ottawa Branch of the Canadian Mental Health Association (CMHA) - Dual Diagnosis Brokerage Service, the JTH will provide a home like environment, clinical treatment, and re-integration supports to the individuals it supports. The JTH is intended to be a rehabilitative and transitional housing option.

This residential program will be staffed at higher ratios in order to meet the complex needs of the individuals served. The JTH provides a combination of individual bedrooms in a group home environment and self-contained 1 bedroom, apartments, professional offices and meeting spaces, group programming, communal eating and recreation space. On-site supports include, personal support with activities of daily living (ADLs), psycho-social rehabilitation programming (including educational upgrading, training and employment supports), healthy meals, life skills training, accompaniment in the community, and a full range of social and recreational activities. Clinical supports include primary health care, psychiatry, psychology, behavioral therapy, and intensive case-management.

As this program is a transitional housing model, a major focus of the program will be to transition individuals to suitable permanent housing in Ottawa or the community of their choice throughout the province of Ontario.

The clinical goals of the program are to:

  • Prevent unnecessary admissions to hospital or jail
  • Prevent unnecessary breaches of probation, parole, or bail conditions
  • Prevent any new charges
  • Assist residents in their recovery from mental illness and re-integration in the community
  • Help residents reach their optimum level of independent functioning
