JOB DESCRIPTION:
Position Summary
The Health Promotion Coordinator plays an integral role in connecting the community with healthcare. They co-ordinate the development, implementation, and evaluation of health education and promotion programs and activities across the lifespan. As a member of the inter-professional health team, they will support the integration of health promotion strategies across primary care and allied health services at PFHT. They champion evidence-based planning in programming and is instrumental at local, regional, and provincial levels with respect to new directions.
Duties and Responsibilities
- Coordinate, develop, implement, monitor and evaluate community health education/promotion programs in response to community needs; supports ongoing domains (e.g. Chronic disease prevention and management, Aging Well, Mental health and social connectedness and Social determinants of health).
- Evaluate processes and collaborates with and/or supports inter-disciplinary teams to assess needs and then support design, implementation and evaluation of complex and sensitive customized programs/activities and quality assurance procedures.
- Lead the planning, implementation and evaluation of health promotion programs for the various target groups by conducting systematic ongoing research/assessment of health education/promotion projects and programs available to our clients/community and in conjunction with key partners.
- Coordinate the delivery of high-quality health education/promotion activities. internally and in partnership with community agencies and services.
- Lead the Communication and Marketing Plan related to the Healthcare Promotion Programs portfolio in collaboration with Management and interdisciplinary team and consultants.
- Coordinates the development of health promotion materials, reports and displays and make recommendations related to health education/promotion. This includes collaborating with staff, volunteers and community agencies/services in the development of promotional materials and activities to ensure that the information is in an appropriate format for the intended audience.
- Contributes to all PFHT’s activities to collect, analyse and report on data and relevant information and participate in research.
- Preserves confidentiality of all client and employee information and seeks to minimize risk while working with data.
- Maintains and develops professional competence through appropriate continuing education methods (e.g. peer interaction, literature review, etc.).
- Complies and adheres to all applicable PFHT policies and procedures.
- Respects and values the diversity of the community and individuals.
- Perform other duties as required.
Education, Skills, and Experience
Education/Training/Certifications:
- Bachelor's degree in Health Promotion or in health science, public administration, recreation administration, hospital administration, social science or a related field.
Previous Related Experience:
- Minimum five years health promotion experience in an interdisciplinary community-based organization.
- Experience with community-based health care and knowledge of community-based resources, health promotion theory, quality improvement and evaluation.
- Experience with conducting needs assessments, gap analysis and program indicator development and evaluation.
- Experience and knowledge of health promotion and education, community organizing techniques, program planning and design.
- Experience with social media, social marketing, web applications.
- Proficiency and experience working in a computerized environment and experience using an electronic health record.
